چکیده

Epilepsy surgery is indicated in select patients with medically refractory focal epilepsy. Seizure freedom or significant reduction of seizure burden without risking new neurological deficits is the expected goal of epilepsy surgery. Typically, when the seizure onset zone overlaps with eloquent cortex, patients are excluded from the surgery. We present a patient with medically refractory frontal lobe epilepsy who underwent successful surgery with resection of the Broca’s area, primarily involving the pars triangularis (BA 45). We document transient expressive aphasia followed by recovery of speech. This case provides new insights into the plasticity of the language network. Presurgical Language Mapping in Patients With Intractable Epilepsy: A Review Study

Introduction: about 20% to 30% of patients with epilepsy are diagnosed with drug-resistant epilepsy and one third of these are candidates for epilepsy surgery. Surgical resection of the epileptogenic tissue is a well-established method for treating patients with intractable focal epilepsy. Determining language laterality and locality is an important part of a comprehensive epilepsy program befo...

The Prognosis and Recovery of Aphasia Related to Stroke Lesion

OBJECTIVE To investigate the effects of specific brain lesions on prognosis and recovery of post-stroke aphasia, and to assess the characteristic pattern of recovery. METHODS Total of 15 subjects with first-ever, left hemisphere stroke, who were right handed, and who completed language assessment using the Korean version of the Western Aphasia Battery (K-WAB) at least twice during the subacut...

Variable fMRI activation during two different language tasks in a patient with cognitive delay.

Functional MRI produces a more accurate localization of the language areas for epilepsy surgery purpose, but requires the patient's cooperation. We report a 34 year-old woman with mental retardation who underwent two different verbal fluency tasks, category and word naming. We found a strong activation of Broca's area in the most difficult task. We suggest that a multi-task fMRI study could be ...

Evidence of cortical reorganization of language networks after stroke with subacute Broca's aphasia: a blood oxygenation level dependent-functional magnetic resonance imaging study

Aphasia is an acquired language disorder that is a common consequence of stroke. The pathogenesis of the disease is not fully understood, and as a result, current treatment options are not satisfactory. Here, we used blood oxygenation level-dependent functional magnetic resonance imaging to evaluate the activation of bilateral cortices in patients with Broca's aphasia 1 to 3 months after stroke...

Reorganization of Language Areas in Patient with a Frontal Lobe Low Grade Glioma – fMRI Case Study

BACKGROUND Functional magnetic resonance (fMRI) studies results in case of an adult patient with low grade glioma (LGG) in dominant hemisphere suggest brain plasticity process with acquisition of language functions by the non-dominant hemisphere speech regions. CASE REPORT A 36-years old right-handed woman was admitted to the Department of Neurosurgery for surgical treatment of brain tumor. A...
